Family Medicine at East Portland is a Federal Qualified Health Center (FHQC) providing primary care. The patient population includes pregnant people, babies, children, adolescents, adults, and geriatrics. The patients are generally moderately stable to moderately unstable, with complex medical conditions and SDOH barriers that require medical and nursing interventions.

This position will cross-train to support additional primary care clinics within the Department of Family Medicine and the Division of Internal Medicine. Responsibilities include remote phone triage, MyChart message management, results review and follow-up, transitions of care, and other duties aligned with operational needs to support clinic workflows and the RN team.

Team based care and interdisciplinary collaboration
Chronic disease management
Foot care clinics
Experience in independent RN triage via phone and in-basket messaging utilizing protocols
Conducts RN led visits within scope i.e. iv placements, medication administration, chronic disease education, wound care, etc
Experience in longitudinal care management and care coordination
Current Basic Life Support (BLS) issued by American Heart Association (AHA)
Length of Orientation – New Graduate Nurse
