We are seeking a dynamic individual to join our team within IDD/CDRC. The Advanced Practice Provider (APP) provides collaborative and multidisciplinary care in the OHSU/CDRC Spina Bifida Program, Feeding and Swallowing Disorders Clinic, and the Autism Clinic with a team of attending physicians, APPs, and allied health professionals. The APP will provide diagnosis, consultation, and treatment (to include care coordination and referral services) to individuals and their families with spina bifida, complex feeding difficulties, and autism. For patients with Spina Bifida, the APP will provide specialty care, especially related to bowel and bladder functioning, family coping, self-care, self-concept, growth and development and consultation to other community agencies and providers. The APP will work within an interdisciplinary model, coordinate clinic activities, and have primary responsibility for case management and care coordination. The APP shall provide services as assigned by the supervisor in furtherance of the university’s missions and goals of teaching, research, patient care, outreach and public service.   The APP faculty will have opportunities to become involved in quality improvement work, education, research, and childhood advocacy. Doernbecher Children’s Hospital, the Department of Pediatrics, and the Division of IDD/CDRC are committed to developing our faculty members’ careers. A pathway for academic promotion for the APPs was recently implemented at OHSU.   Professional participation is expected including attendance in relevant educational conferences and didactic conferences. Perform initial Assessments and initiate Care Plans as required by policy and for IDS Members as required by the contract held between OHSU Health Services and Health Share of Oregon and within the timelines established. Reassessment of Member status and adjustment of care plans completed to meet Member on-going needs and goals of care. Monitor and evaluate services and community-based resources necessary to respond to the Member’s individual healthcare needs.   3. Provide Member care coordination and self-management through face-to-face and telephonic outreach, which may include home visits, attending clinic office visits, and interaction with patients being seen in the ED or who are in the hospital. These interventions are intended to allow the opportunity to assess the Member’s ongoing needs and to identify resources that will help ensure the Member is meeting his/her health care goals.   4. Collaborate with OHSU Health Services Member’s medical and healthcare community-based providers (that may include the primary care provider (PCP), specialist providers, behavioral specialists, dental providers, or others that may be indicated) regarding the Member’s treatment needs and plan of care. Serve as a key link between the Member, caregivers and the multi-disciplinary clinical team. The frequency of contact is determined based on the Member’s risk acuity ranking and clinical need.    5. Routinely document all Member interactions and Member engagements in the electronic medical record (EMR) to ensure a longitudinal record is maintained of all Member interactions. Collect clinical engagement data and enter into the EMR and/or other indicated database for tracking and metric purposes. Maintain medical record confidentiality at all times through the proper use of computer passwords, maintenance of secure files, adherence to HIPAA guidelines and policies.   6. Ensure day-to-day processes are conducted in accordance with nationally accepted standards (NCQA, URAC, and other regulatory standards) and for IDS Members, the contract held between OHSU Health Services and Health Share of Oregon. Participate in quality and performance improvement activities that will look to continually enhance the effectiveness, efficiency and quality of services being provided.   7. Utilizes proper telephone and personal etiquette and judicious use of other verbal and written communications, following OHSU and OHSU Health Services policies, procedures and guidelines. The NN ensures that when hospitalization is required; those patients and their family members understand prior to admission, what they can expect during the pre-hospitalization, hospital stay and post discharge period.  The NN works with the care coordination team and inpatient team on the admission and discharge plan. The NN will work with the orthopedic total joint APPs on triage of patient specific issues and concerns pre and post surgery. Accountable for the key performance measures of the joint replacement patient and works with the Quality team to identify and track key performance measures, including but not limited to: Patient satisfaction, physician satisfaction, LOS, variable cost/case, discharge disposition, and orthopedic specific outcome tools as necessary. The nurse navigator coordinates the care for the patient prior to the patient’s first consult visit at OHSU.  This position will work with sarcoma team to develop standard metrics, identify patient barriers, coordinate smooth transitions of care, and to be available for team members in gathering data for new patients that will allow the team to create a plan of care at the patient’s first visit and, through the work up phase of their disease. An Oncology Nurse Navigator (ONN) at OHSU is a professional RN with oncology-specific clinical knowledge who offers individual assistance to patients, families, and caregivers to help overcome healthcare system barriers.  Using the nursing process, an ONN provides education and resources to facilitate informed decision-making and timely access to quality health and psychosocial care as they approach their first visit.   This individual will serve as the primary point of contact for all navigated patients within the disease specialties they cover and will thoroughly hand off care (verbally and in EMR documentation) to providers and nurses assuming care of the patient. The hand off point varies by specialty. In some cases, the hand off may occur after the first consult, but for other patients, the navigator will follow the patient through initial modalities of care, particularly when patients need monitoring for timeliness in transitioning to the next modality of care or adherence to treatment standards (i.e., transition from surgery to med onc, med onc to rad onc). The ONN interfaces with research staff, and works to identify patients who may be suitable candidates for clinical trials

The Research Nurse is responsible for the clinical research activities for cardiomyopathy clinical research studies conducted at the OHSU Knight Cardiovascular Institute in the Department of Cardiology. The Research Nurse works collaboratively with other staff and provides services, as assigned by the clinical trials leadership team and/or the cardiomyopathy section head, in furtherance of the university’s mission and goals. The primary objectives of this position are to recruit, screen, monitor and document the subjects’ responses to clinical trial interventions. The Research Nurse provides monitoring and clinical expertise to ensure protocol adherence and accurate data collection. The Research Nurse will be responsible for administering investigational products through different routes including intravenous infusions, and in certain cases gene therapy or gene editing investigational products. In addition to direct research responsibilities, the individual in this position is responsible for mentoring OHSU or affiliate site staff members, and identifying opportunities for and implementing process improvement.    The research nurse will have a very strong understanding of research regulations, reporting requirements, and EMR documentation for research purposes.    This position will primarily support our Hypertrophic Cardiomyopathy research team within the division of Cardiology, they may be reassigned secondary duties to support other sections within the Cardiovascular Medicine Clinical Trials Unit and Knight Cardiovascular Institute.

Patients may frequently present with alcohol or drug abuse history, and complex medical histories. The ED Team works determine if the patient requires admission or discharge with resources and referrals.
