We are seeking a clinic RN Coordinator that provides professional, comprehensive nursing care and coordination for this complex surgical patient population.  The RN Coordinator will provide care coordination, assessment and triage, medication management and clinical support to the surgery faculty providers in the clinic setting. With faculty support, the RN coordinator will require independent critical decision making skills to assess complex patient population.   Colorectal: - Review endoscopy path reports, discuss with providers and update patient/referring MD - Review unresectable polyp referrals to determine best provider/place/time Abdominal Wall: - Review undirected referrals to determine best provider / place / time Foregut: - Support esophageal tumor board conference by reviewing outside documents in a timely manner HPB: - Provide vacation / FMLA / sick leave as noted below Bariatric - Provide vacation / FMLA / sick leave as noted below Provides resource coverage for RN – as needed for vacations / extended leave / breaks  - The RN Coordinator collaborates with other members of the RN team to ensure that patient flow is expedited during clinical practice sessions - Coordinates and implements initiation of treatment plan of care - The RN Coordinator ensures that Provider orders that result from the new patient visit are recorded and executed.  S/he also works to ensure that laboratory and other diagnostic findings are available to clinicians in a timely fashion after tests are performed.  S/he tracks the post visit flow information to ensure that plans of care are executed in a timely fashion - Triages and responds or refers inquiries appropriately.  Recognizes and reports specific physical and emotional conditions/needs/concerns of the patients to appropriate members of the health care team.  Alerts Providers to unusual observations of patients to facilitate immediate attention - Triages all urgent patient calls; triages Provider calls and assists with communication when necessary. The General GI Care Coordinator is a member of a multi-disciplinary team with primary responsibility for direct and indirect professional nursing care of medicine patients that have complex gastrointestinal diseases.  The patients managed by this team are medically complex with functional GI disorders and nutritional deficiencies requiring supplemental nutrition, including total parental nutrition. Other complex and chronic GI diseases, requiring long term, longitudinal care from their gastroenterologist comprise much of this patient population.   - - Responsibilities include the care coordination and case management of individuals in the outpatient clinic. - S/he will provide clinical support services to faculty and APP in the Gastroenterology Division. - The nurse Coordinator will be responsible for coordinating comprehensive care for patients and will be a key contributor to ensuring that patients are guided through all stages of their treatment at OHSU. This patient population can be labor intensive given their complex diagnoses and multi-disciplinary care. This translates to increasingly complex care coordination. - The nurse coordinator will be the core of a multi-disciplinary patient care team, which includes physicians, APP, RN, MA, and PAS-R support. - The nurse coordinator will have specialized knowledge and skills in complex gastroentestinal disease management. - This position serves as a coordinator for patients undergoing sequenced multidisciplinary treatment, and at times patients who travel from long distances from home. - Responsibilities include but not limited to care coordination, patient assessment, triage, patient education, medication management, discharge and admit planning, and clinical support for the GI clinic faculty providers. - This position will also participation in quality improvement, medical record review and  will follow existing protocols determining appropriate patient scheduling.  - Must be able to deal with competing needs and priorities - Requires ability to triage and assess complex patient symptoms/condition - Requires excellent time management skills

Provides pre- and post-op education, educational resources and support to patients and their families. The NN acts as a resource to all members of the health care team related to provision of care, enhancing assessment skills of others, establishment of appropriate care standards, and determination of realistic goals for optimal patient outcome. The NN ensures that when hospitalization is required; those patients and their family members understand prior to admission, what they can expect during the pre-hospitalization, hospital stay and post discharge period.  The NN works with the care coordination team and inpatient team on the admission and discharge plan. The NN will work with the orthopedic total joint APPs on triage of patient specific issues and concerns pre and post surgery. Accountable for the key performance measures of the joint replacement patient and works with the Quality team to identify and track key performance measures, including but not limited to: Patient satisfaction, physician satisfaction, LOS, variable cost/case, discharge disposition, and orthopedic specific outcome tools as necessary. The nurse navigator coordinates the care for the patient prior to the patient’s first consult visit at OHSU.  This position will work with sarcoma team to develop standard metrics, identify patient barriers, coordinate smooth transitions of care, and to be available for team members in gathering data for new patients that will allow the team to create a plan of care at the patient’s first visit and, through the work up phase of their disease. An Oncology Nurse Navigator (ONN) at OHSU is a professional RN with oncology-specific clinical knowledge who offers individual assistance to patients, families, and caregivers to help overcome healthcare system barriers.  Using the nursing process, an ONN provides education and resources to facilitate informed decision-making and timely access to quality health and psychosocial care as they approach their first visit.   This individual will serve as the primary point of contact for all navigated patients within the disease specialties they cover and will thoroughly hand off care (verbally and in EMR documentation) to providers and nurses assuming care of the patient. The hand off point varies by specialty. In some cases, the hand off may occur after the first consult, but for other patients, the navigator will follow the patient through initial modalities of care, particularly when patients need monitoring for timeliness in transitioning to the next modality of care or adherence to treatment standards (i.e., transition from surgery to med onc, med onc to rad onc).
